Self-referral
To become a patient of Carolina Pain Physicians complete the forms in the links above. You may need to fill out multiple medical releases if you have been previously treated at more than one facility. Once complete fax, mail, or email all the forms to our office. We will then send the releases to your previous providers. Upon receiving the records they are sent to our intake team for review. Our intake team will then call you to set up an appointment.
